According to the law "On Compulsory Health Insurance in the Russian Federation" dated November 29, 2009 No. 326-FZ, citizens have the right to replace their medical institution once a year with another one that, in their opinion, suits them better. Or several times - in case of a change of residence. The transfer procedure from one clinic to another must be carried out before November 1 of the current year.

To register with the selected clinic, you need to appear there in person or send a representative with an appropriately executed power of attorney. In the registry, inform about your intention and, having received the application form, fill it out. After verifying the information you provided in the application, the staff of the selected medical institution will notify you of the attachment. In accordance with the law, if the polyclinic changes, it is not necessary to detach from the previous medical institution, the employees themselves will request the necessary documentation.

To attach to the clinic at the actual place of residence, you need to bring with you:

• the passport;

• policy.

If a child under the age of 14 is transferred to another clinic, then:

• birth certificate;

• child policy;

• parent's passport.

Foreign citizens permanently residing in Russia, in order to be transferred to another clinic, must provide:

• identification;

• resident card;

• policy.

Foreigners temporarily staying in Russia provide for translation:

• identification;

• policy.

If the change of the clinic is associated with a change of residence, you need to take with you a document confirming such a change. This may be a contract of employment, an air or railway ticket, and more.
